Preparing for Your Visit:
Please remember to bring the following:
- Your insurance cards
- A list of current medications
- Your glasses, prescription sunglasses, or if you are a contact lens wearer, a box or packet of contacts for each eye that you are presently wearing
- If your insurance requires a referral, please obtain this from your primary care physician’s office prior to your visit
- Payment for any portion of the exam not covered by your insurance and co-pays are to be paid at the time of the exam
